According to the ADA more the 85% of the population have some kind of gum disease. unfortunately silent and painless periodontal disease can progress undetected until the gums start receding and bleeding destroying the underlying bone and attachments. Patients usually complain of bad breath. The goal of modern dentistry is to prevent tooth loss. When a tooth cannot be saved, extraction (removal) of the tooth may be the only course of treatment. A panorex x-ray allows us to see a complete picture of the alignment of your jaw, the positioning of your teeth and if there is sufficient space for your third molars (wisdom teeth) to fully erupt. In addition, it is an excellent diagnostic tool for evaluating the supporting bone for possible cysts or tumors.

When an untreated cavity reaches the nerve, it becomes infected or inflamed. The root canal treatment may be the only way to save the tooth, short of removing it. Root canal treatment consists of removing the diseased nerve from within the canal space. Most root canals take one visit, infected ones can take several visits. After root canal therapy, your dentist may refer you to a specialist ( prosthodontist) to restore the tooth with a post and a crown.

Dr. Mokbel's Artistic side
For many dentists dentures are not what they look for, but with many years of experience and teaching I find a great pleasure restoring some ones smile and confidence, many of my graduate student from USC refer their own friends and family. Full and partial denture require a lot of skills, precision and patience as well as understanding the physiology and the anatomy of the aging structures of the mouth and the jaw. Removable Partial dentures are held in the mouth with metal clasps around some of the remaning teeth, they can be very comfortable when done precisely, and all of our patients wear them without any problems. Incorporating precision attachment into a cap can eliminate the ugly unpleasant clasp.
Some new all soft plastic partials have been marketed as a low cost, more esthetic, replacement of these traditional partials, But what they fail to tell the consumers, and the general dentist who sell them, is that they are temoporary and when used for long term they can cause drifting of the remaing teeth and affect the bite. As an educator I do not recommend those kind of partials and we do not recommend them to our low income patients at the dental schools.
When dealing with lower dentures especially, many patients can experience difficulties in keeping their dentures in place, It is the standard of care to recommend at least 2 implants to hold the lower denture. This will make the patient more comfortable, the more implants the better.

Actual cases of Dr. Mokbel's
The lower jaw is connected to the base of the head with the temporo-mandibular joints. The teeth help guide the condyles in their place inside the joints, when the condyles don't seat in place properly they can cause a disease known as the "TMJ syndrome". Many patients can experience clicking and popping of the joints associated with different degrees of pain and discomfort. Grinding teeth also known as "Bruxism" can contribute and exacerbate these symptoms. Many patients can grind their teeth down to the nerve.
Treatment with different kinds of appliance such as an "occlusal guard" can bring a lot of relief. Occlusal guards must be done in a certain way unfortunately the occlusion of the teeth and joints varies a lot between patients, the importance of the muscles and the nervous system must be evaluated, and the treatment should be personalized.
We always check for symptoms during our examination and make sure that any restoration we fabricate will not interfere with the normal function of the joints.
A thorough and a more complexe examination usually include palpation of the joints and the muscles, examination of the surfaces of the teeth, evaluating the Bite and the mouvements of the articulation, Listening to the noises in the joints with a Doppler machine, taking impressions of the teeth and mounting the models on a Jaw similator or articulator. In most cases a CT Scan is helpful. Gathering data is extreemly important in determining the course of action.
Treatment can be limited to just an Occlusal Guard or Adjusting the Bite, A large emphasis in Prosthodontics is placed on understanding the way the jaws function and the teeth meet, and many patient may have all their teeth restored to achieve a balance between the joints, the muscles and the nervous system. In some cases Surgery is the only resort.

Replacing large fillings in back teeth often requires stronger, more precisely contoured restorations. These are called inlays or onlays depending on
whether they are being placed inside the tooth or on its outer surface. These restorations need to be custom-made at an off-site dental lab; therefore,
two visits may be required. Once the dental office receives the custom inlay or onlay, the patient returns to the dentist's office, where the restorations are bonded into place.

Considered one of the safest and most effective methods of tooth whitening, the Zoom! procedure takes approximately one hour.
Zoom! whitening begins when your dentist applies a custom whitening gel containing ph-balanced hydrogen peroxide to the patient’s teeth.
Next, a special light will be directed towards the teeth, activating the whitening gel. The light will remain directed at the teeth for approximately
one hour. Following the procedure, the patient’s teeth will be dramatically whiter.
Take-home Tooth Whitening
These kits contain a customized tray that is made from custom molds of your teeth and whitening gel made of hydrogen peroxide. The patient usually

Patient Concerns
When using the at-home treatment, patient's teeth may feel a heightened, but temporary, sensitivity. The gums may also feel more sensitive or burn

Birth to 6 months of age:
Clean the infant's mouth with gauze after feedings and at bedtime.
Consult your child's pediatrician regarding fluoride supplements.
Regulate feeding habits (bottle feeding and breastfeeding).
Six to 12 months of age:
During this time, the first tooth should appear. Consult the Pediatric
dentist for an examination. Brush teeth after each feeding and at bedtime with a small, soft-bristled brush.
As the child begins to walk, stay alert of potential dental and/or facial injuries. Wean the child from breast or
bottle by his/her first birthday.
Twelve to 24 months of age:
Follow the schedule of dental examinations and cleanings, as recommended by your child's pediatric dentist. Generally, dental examinations and cleanings
are recommended every 6 months for children and adults.
As your child learns to rinse his/her mouth, and as most deciduous (baby) teeth have erupted by this age, brushing with a pea-sized
portion of fluoridated toothpaste becomes appropriate.
Facts about deciduous teeth:
Proper care of a child's deciduous teeth (also known as "baby" or primary teeth) is very important as these teeth hold space for the future eruption
of permanent teeth.
If a baby tooth decays or is removed too early, the space necessary for the permanent teeth is lost and can only be regained through orthodontic
treatment.
Infected baby teeth can cause the permanent teeth to develop improperly resulting in stains, pits, and weaker teeth.
Most children begin losing their baby teeth around the age of 4 - usually the front bottom teeth first. They continue to lose baby teeth until
the age of 12 or 13 when all of the permanent teeth finally come through.
Thumb-Sucking and Dental Health
Generally, thumb-sucking before the age of two is normal and harmless. When thumb-sucking is not stopped by the appropriate age
(generally by the age of five) then parents should discourage the act. Prolonged thumb-sucking may contribute to crowded and/or crooked teeth
development and bite problems.
Diet and dental care for children:
The American Academy of Pediatric Dentistry recommends the following to
ensure your child eats correctly to maintain a healthy body and teeth:
Ask your pediatric dentist to help you assess your child's diet.
Shop smart. Do not routinely stock your pantry with sugary or starchy
snacks. Buy "fun foods" just for special times.
Limit the number of snack times; choose nutritious snacks.
Provide a balanced diet, and save foods with sugar or starch for mealtimes. Do not put your young child to bed with a bottle of milk, formula,
or juice. If your child chews gum or sips soda, choose those without sugar.
For Americans in their 30s and beyond, the threat of gum disease (periodontal disease) is a very real and
potentially dangerous condition. Gum disease is particularly dangerous because the progression of the
disease is often painless, going undetected until it creates serious problems.
Although genetics may play a small role in its development, doctors agree that gum disease is most often
directly related to how well people care for their teeth and gums.
The Stages of Gum Disease
Periodontal disease is a gradual infection of the gums and the supporting bone. It is caused when the build
up of plaque on and around the teeth calcifies into tartar. This tartar is covered with plaque that contains
micro- organisms which release toxins causing the first stage of gum disease known as gingivitis. Gingivitis
is characterized by swelling, inflammation, and bleeding of the gums. In the advanced stage of gum disease
(referred to as periodontitis), the toxins cause the bone to resorb or dissolve. The teeth having lost a
part of their bone support begin to move & eventually with more bone loss there is tooth loss.
The Surgery
Treatments to alleviate the effects of gum disease include deep cleaning, localized delivery of antibiotics,
home care & regular follow up care. The periodontist / Dentist begin by removing the diseased tissue, tartar,
and plaque from the tooth's surface and below the gum line. The root of the tooth may need to be planed and
smoothed in order to allow gum tissue to heal properly. The periodontist may also advise bone grafts
depending on the amount of bone that is lost due to gum disease.
Prevention

The goal of modern dentistry is to prevent tooth loss. When a tooth cannot be saved, extraction (removal) of the tooth may be the only course of treatment. A panorex x-ray allows us to see a complete picture of the alignment of your jaw, the positioning of your teeth and if there is sufficient space for your third molars (wisdom teeth) to fully erupt. In addition, it is an excellent diagnostic tool for evaluating the supporting bone for possible cysts or tumors. When an untreated cavity reaches the nerve, it becomes infected or inflamed. The root canal treatment may be the only way to save the tooth, short of removing it. Root canal treatment consists of removing the diseased nerve from within the canal space. Most root canals take one visit, infected ones can take several visits. After root canal therapy, your dentist may refer you to a specialist ( prosthodontist) to restore the tooth with a post and a crown. Dr. Mokbel's Artistic side For many dentists dentures are not what they look for, but with many years of experience and teaching I find a great pleasure restoring some ones smile and confidence, many of my graduate student from USC refer their own friends and family. Full and partial denture require a lot of skills, precision and patience as well as understanding the physiology and the anatomy of the aging structures of the mouth and the jaw. Removable Partial dentures are held in the mouth with metal clasps around some of the remaning teeth, they can be very comfortable when done precisely, and all of our patients wear them without any problems. Incorporating precision attachment into a cap can eliminate the ugly unpleasant clasp.Some new all soft plastic partials have been marketed as a low cost, more esthetic, replacement of these traditional partials, But what they fail to tell the consumers, and the general dentist who sell them, is that they are temoporary and when used for long term they can cause drifting of the remaing teeth and affect the bite. As an educator I do not recommend those kind of partials and we do not recommend them to our low income patients at the dental schools. When dealing with lower dentures especially, many patients can experience difficulties in keeping their dentures in place, It is the standard of care to recommend at least 2 implants to hold the lower denture. This will make the patient more comfortable, the more implants the better. Treatment with different kinds of appliance such as an "occlusal guard" can bring a lot of relief. Occlusal guards must be done in a certain way unfortunately the occlusion of the teeth and joints varies a lot between patients, the importance of the muscles and the nervous system must be evaluated, and the treatment should be personalized. We always check for symptoms during our examination and make sure that any restoration we fabricate will not interfere with the normal function of the joints. A thorough and a more complexe examination usually include palpation of the joints and the muscles, examination of the surfaces of the teeth, evaluating the Bite and the mouvements of the articulation, Listening to the noises in the joints with a Doppler machine, taking impressions of the teeth and mounting the models on a Jaw similator or articulator. In most cases a CT Scan is helpful. Gathering data is extreemly important in determining the course of action. Treatment can be limited to just an Occlusal Guard or Adjusting the Bite, A large emphasis in Prosthodontics is placed on understanding the way the jaws function and the teeth meet, and many patient may have all their teeth restored to achieve a balance between the joints, the muscles and the nervous system. In some cases Surgery is the only resort. |
